There is a strict procedure for the management of information relating to cases of academic misconduct.
The use of a uniform recording system across all faculties in relation to cases of academic misconduct is a central feature of the Academic Conduct Guidelines for Faculties and Other Teaching and Supervision Sections at UWA: Ethical Scholarship, Academic Literacy and Academic Misconduct.
A uniform system is designed to introduce a more streamlined approach to the handling of cases, and to underpin simultaneously the University's development and support of academic literacy among its students.
The appropriate capture and management of information relating to cases of academic misconduct:
For these reasons it is important that relevant information is recorded in all cases of suspected misconduct, whether minor, moderate or major.
Where any incidents of academic misconduct are suspected, the Academic Misconduct Investigation and Reporting Proforma must be completed and forwarded immediately upon completion to the Records Coordinator.
The completed form and any supporting documentation should be placed in a sealed envelope marked:

Once the form and supporting documentation have been received it will be scanned, allocated the Student in Confidence caveat and attached electronically to the student’s electronic student file in TRIM.
Only staff with access to the Student in Confidence caveat will be able to see this documentation. Staff without the appropriate security clearance will see only unclassified student documentation and a note on the file’s registration stating “This student file contains restricted documentation”.
Where any amendment to a decision about academic misconduct occurs as a result of appeal, the student's record will be amended to reflect such outcomes. Any findings of academic misconduct attached to a student's record will not at any time appear on a student's official transcript.
This procedure will ensure both students' privacy, and that the handling of a case is not prejudiced by access to such information prior to any case being proven.
Information relating to academic misconduct will be retained for the same period as the full electronic student file. In most cases, the electronic student file is retained for 10 years after the student graduates and is then destroyed in accordance with an approved Disposal Authority.
